Many degenerative neutrophils were seen together with some acatholytic keratinocytes.

Acatholytic keratinocytes can be suggestive of pemphigus complex, however you may note those acantholytic cells in pustular dermatophytosis or staphyloccal pyoderma.

One may put pemphigus high in the differential list if normal-looking intact neutrophils and acatholytic cells found in cytology without free bacteria. But again you may need to rule out dermatophytosis by fungal culture.

I always make the diagnosis of pemphigus ONLY AFTER dermatohistology confirmation with appropriate clinical findings.
